New
students who require accommodations from the Office
for Students with Disabilities should follow these
steps in order to register:
Step 1
Obtain an Application for Support Services. You
can get the form by clicking
here. Fill out the form, print it, and include
your signature in the appropriate area.
Step 2
Obtain official documentation of your disability.
Step 3
Submit your documentation to one of the OSD office
locations. You can find the locations by clicking
here.
Step 4
Meet with an OSD Counselor in person for an intake
interview.
